Patient Information

Office Hours

Our regular office hours are Monday through Friday 8:00 am to 5:00 pm.

Emergencies

We provide 24-hour emergency care for our patients. If you are experiencing an emergency after office hours, please call our office number and you will be directed to our physician on call, who will get back to you promptly to treat your condition and provide the urgent care required. In the case of a true emergency, please call 911 or go to your nearest emergency room.

Payment Options

Credit Cards

We accept cash, check, Visa, MasterCard, American Express, Discover and Debit cards.

We also offer financing through CareCredit. Please call our office for more information regarding financing or visit www.carecredit.com.

Insurance

We participate with most major insurance providers. Please check with your individual provider to make sure that your services will be covered under your specific plan. While our staff will be happy to assist you with any insurance questions you may have, it is inevitably the responsibility of the patient to verify eligible coverage from their provider.

Cancellation Policy

We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24-hour notice. Please extend us this courtesy so that other patients may be seen.

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

  • Patient intake form including sections for responsible party information, insurance information, and HIPAA medical release.
  • Advance Beneficiary Notice of Noncoverage if in the event Medicare/Managed Care doesn’t pay for specific services listed on the form, then it is the responsibility of the patient for payment.
